In the midst of this terrible pandemic, a beacon of hope now shines out from Uganda. This does not look like much, but it had a profound impact on the prevalence of HIV/AIDS and it also demonstrated the huge cultural changes that young girls were able to abstain. For boys, it rose from 17.6 to 18.3 years in the second half of the decade.

For youth, the A of the ABC message, abstinence was probably the most important single message. Many people, including Uganda's first lady, acknowledged that the B, be faithful, which includes partner reduction, may be overall the most important factor. While C, condom promotion was not a dominant element in Uganda' s early response to AIDS, (mainly as it affronted the teaching of the Catholic Church), there has been some turn of heart in more recent years and with it, increased condom use. There is a definite beneficial role of condoms when one spouse is HIV positive and the other is not but it must also be seen as an important prevention tool for people who have sex with multiple partners. In Sub-Saharan Africa, HIV is mainly transmitted through heterosexual intercourse. Variances in sexual behaviours across cultures, age groups, and gender usually influenced by culture and social, economic circumstances influence HIV prevention interventions. Dr David Serwadda, a doctor who treated the first AIDS patient in Uganda, says two factors have been key in his country's turnaround. People have cut down the number of sexual partners and young people are starting to have sex at a later age (15.5 years rather than 14).

Dr Coutinho feels the key factor is individual responsibility. He recently told me that he has lost nine close relatives and takes care of two AIDS orphans. "The disease has touched us all!."

If we were realistic, not only is no one in Uganda untouched by this horrific illness, but the difference is that there is acceptance in Uganda that AIDS is the cause of death - rather than the myriad of other causes usually listed to obscure AIDS deaths, as happens in South Africa. This openness is in large part thanks to the fact that Uganda's President Yoweri Museveni has made HIV/AIDS one of his three personal priorities, alongside peace and economic empowerment, for the past 16 years. In an environment where the country's first citizen speaks about HIV every day, it is no wonder that the country's most famous pop star, as well as army officers and an Anglican priest, have felt able to announce to the nation that they were HIV positive.

President Museveni's commitment has created a climate for a unique partnership between the Ugandan government, civil society, and international donors that has led to the formation of a multi-sectoral AIDS Commission to drive AIDS prevention. Dr Coutinho says it is important that Uganda does not rest on its laurels. There are still many challenges, including new infections in the rural areas and only about 5% of pregnant HIV positive women have access to anti-retroviral drugs that prevent them from infecting their babies.

The top military commander

The top military commander in Afghanistan warns in a classified document that more US forces are needed within the next year or the mission ‘will likely result in failure,’ the Washington Post said Monday.

General Stanley McChrystal, the top US inflatable and NATO commander in the country, writes in a grim assessment of the eight-year conflict: ‘Failure to gain the initiative and reverse insurgent momentum in the near-term (next 12 months) -- while Afghan security capacity matures -- risks an outcome where defeating the insurgency is no longer possible.’

The document, first obtained by the Post, was presented to US Defense Secretary Robert Gates on August 30 and is currently being reviewed by the White House.

In some of the most alarming passages of the report, McChrystal, who has been widely expected to submit a formal request to increase the 62,000-strong inflatable bouncer US force, noted the campaign in Afghanistan ‘has been historically under-resourced and remains so today.’

As such, he wrote, ‘[I]nadequate resources will likely result in failure.’ Inability to provide adequate resources, he writes, ‘also risks a longer conflict, greater casualties, higher overall costs, and ultimately, a critical loss of political support. Any of these risks, in turn, are likely to result in mission failure.’ The 66-page document -- a declassified version of which is published at washingtonpost.com -- describes a strengthening, intelligent enemy in the Taliban insurgency.

McChrystal also slams the corruption-riddled Afghan government and a strategy by international forces in the country that has failed to win over the civilian population.
‘The weakness of state institutions, malign actions of power-brokers, widespread corruption and abuse of power by various officials, and (the International Security Assistance Force's) own errors, have given Afghans little reason to support their government,’ wrote McChrystal.

International forces, he said, ‘have operated in a manner that distances us -- physically and psychologically -- from the people we seek to protect... The insurgents cannot defeat us militarily; but we can defeat ourselves.’ The general, who inflatable water games Gates nominated to take over operations because his ‘new thinking’ was needed as President Barack Obama attempts a new strategy for the war-torn country, also warns that hardline insurgents reach systematically into Afghanistan's bloated prison system for recruits.

Whatever happens in the short run, the commander warns it ‘is realistic to expect that Afghan and coalition casualties will increase.’ Obama weighed in Sunday on the debate over more troops in Afghanistan, as he digests McChrystal's report.
